An after-action review format for executive teams

A proposed independent retrospective that turns a difficult decision into owned learning rather than blame or performative toughness.

Make the decision visible

Teams often remember different versions of what was agreed. Start with a concise, factual account: objective, information available at the time, decision made and intended effect. Avoid rebuilding history as if the team knew everything that emerged later.

02 / 05

Separate the result from the quality of the process

A good decision can produce a disappointing result and a weak process can sometimes get lucky. Ask whether the assumptions were reasonable, who had relevant information and how dissent was handled. The purpose is to learn, not find someone to punish for a hindsight-perfect answer.

03 / 05

Give quieter contributors a way in

A facilitated review should allow written reflection before group conversation, especially where seniority can inhibit useful disagreement. The team should agree confidentiality boundaries and how feedback will be used. It should not collect sensitive employee or client information through an ordinary website enquiry.

04 / 05

Choose a small number of owned changes

If the session produces fifteen vague commitments, little may change. Select a few specific actions: a clearer escalation threshold, a one-page options record or an agreed owner for critical assumptions. Define when someone will review whether the change was useful.

05 / 05

Commission responsibly

This programme is at concept stage. A paid engagement would require a competent civilian facilitator, scope, fee, data protection arrangements and an honest statement of what is and is not being assessed. It has no official specialist military connection.

What to specify in the pilot

Ask each participant to complete four headings anonymously before the group meets: expected outcome, actual outcome, assumption we made, improvement I can own. Compare patterns rather than personalities.
